Levels of Care

Suites at Rouse: Levels of Care Assessment

To ensure quality care, each Suites at Rouse resident is assessed upon admission and periodically throughout their stay to determine the appropriate level of care services required to meet their individual needs. These services are provided on three levels:

 

Care Image

Medication Management

Included in base rent: Administration of up to 10 routine medications as ordered by your primary care provider (PCP), excluding PRN (as-needed medications).

  • Administration of 11 or more scheduled medications by staff
  • Routine nebulizer treatments (excluding PRN)
  • Specialized medication preparation (e.g., crushing)
  • Daily insulin administration, sliding scale insulin, and/or routine blood glucose monitoring
  • Vital sign monitoring for specific medications or if ordered more than weekly
  • Administering two or more eye drops more than twice daily
  • Staff assistance with oxygen use, including obtaining supplies or reminders
  • Daily (basic) wound care for longer than 2 weeks

Dietary Assistance

Included in base rent: Help with meal setup, opening containers, pouring liquids, and incidental assistance, where the resident completes at least 75% of the activity.

  • Moderate to maximum assistance with eating, including reminders and cueing
  • Frequent room meals (more than 4 times per week)
  • Assistance with getting to and from meals, including walking assistance or transport in a wheelchair

Physical Care

Included in base rent: Setup, supervision, and cueing for grooming, bathing, showering, and dressing. The resident completes at least 75% of the activity. Minimal assistance with ambulation.

  • Moderate assistance with activities of daily living (ADLs) where the resident completes less than 75%
  • Moderate transfer assistance (1-person assist)
  • Moderate assistance with ambulation (1-person assist)
  • Support for managing challenging behaviors (e.g., refusal of care/meds, wandering, or other behaviors)

Continence Management

Included in base rent: Minimal assistance with toileting, where the resident completes 75% of the activity.

  • Moderate assistance with scheduled toileting
  • Enhanced laundry and housekeeping due to incontinence (more than once per week)
  • Staff assistance with catheter or colostomy care

Care Level Scoring

Your care level is determined by the number of items checked. Based on your score, we’ll determine the appropriate level of care to ensure the best support.

0-1 Checks: Care Level I
2-3 Checks: Care Level II
4-5 Checks: Care Level III
6 or more Checks: Care Level IV

If your score is 6 or more, we will evaluate your care needs periodically and, if necessary, discuss options like Skilled Nursing Facility or other placements.
